How To Change Your Coverage
Here's what to do if you want to change your Medicare coverage during an available enrollment period:
If you want to switch to: | This is what you should do: |
Another Medicare health plan (with Health Partners Medicare or another Medicare plan) |
Enroll in the new plan. You will automatically be disenrolled from your current Health Partners Medicare plan when your new plan's coverage begins. |
Original Medicare with a separate Medicare prescription drug plan. |
Enroll in the new Medicare prescription drug plan. You will automatically be disenrolled from your Health Partners Medicare plan when your new plan's coverage begins. |
Original Medicare without a separate Medicare prescription drug plan.

Send us a written request to disenroll. Call Member Relations at 1-866-901-8000 (TTY 1-877-454-8477) if you need help on how to do this. Or contact Medicare at 1-800-MEDICARE (1-800-633-4227), 24 hours a day, seven days a week, and ask to be disenrolled. TTY users should call 1-877-486-2048. You will be disenrolled from your Health Partners Medicare plan when your coverage in Original Medicare begins. |
To update your member information, call Member Relations at 1-866-901-8000 (TTY 1-877-454-8477). From October 1 to March 31, we’re available 8 a.m. to 8 p.m., 7 days a week. And from April 1 to September 30, we’re available 8 a.m. to 8 p.m., Monday to Friday.
