If You Need Help with Expenses in 2019

If you have low income and/or limited assets, you may qualify for help from a variety of programs.

For example, the federal government estimates that about one of every three Medicare recipients qualifies for Extra Help (also known as the Low Income Subsidy) with some or all prescription drug coverage costs, including monthly premiums, deductibles, copays and coinsurance.

If you get Extra Help to help pay for your Medicare prescription drug plan costs, your monthly plan premium may be lower than what it would be if you did not get Extra Help (depending on your plan choice). The amount of Extra Help you get will determine your total monthly plan premium as a member of our plan.

The premium for Health Partners Medicare Prime and Special includes coverage for both medical services and prescription drugs.

If you have questions about Extra Help, you can call Health Partners Medicare at 1-866-901-8000, 24 hours a day, seven days a week. You can also call Medicare or Social Security at the numbers below, or visit the Social Security website for more information.

How to apply

  • Visit the Social Security website to apply online.
  • Call Social Security at 1-800-772-1213 (7 a.m. to 7 p.m., Monday through Friday) to apply by phone or get a paper application. TTY users please call 1-800-325-0778.
  • Visit your local County Assistance Office. Visit www.medicare.gov/contacts or call 1-800-MEDICARE (1-800-633-4227) (TTY 1-877-486-2048) to get the phone number.
